Full Job Description

This is an exciting opportunity to join the Wirral Enhanced 0-19 service as one of the specialist community nurses working with vulnerable 14-19 year olds. The successful candidate will be joining a team who work with compassion and tenacity using a trauma informed approach to work with young people who are open to Children's Social Care, who are NEET and there are concerns relating to risk of and current criminal exploitation, child sexual exploitation, criminal activity, gangs, county lines and modern day slavery.
Using behavioural change theory alongside a therapeutic approach, the nurse will work collaboratively with each of the clients on their caseload of young people to move along a 'journey of change' to support best possible outcomes for them.
They will work closely with other agencies in the contextual safeguarding arena to support clients holistically ., to support the clients on their caseload to identify any health and/or emotional needs, including those related to substance misuse, that are preventing them fulfil their potential.

see job description for more detail., To develop the 0-19 Health and Wellbeing services working in partnership with the Local Authority, partner agencies and Primary Care Team.
To provide a holistic, health and wellbeing service to vulnerable young people aged 14 - 19 years who meet service eligibility criteria to include involvement or at risk of CSE/CE, modern day slavery and/or criminality.
To develop and deliver health care and health promotion as a member of the 0-19 services, using evidence based practice and professional leadership and management.
To contribute to the development of best practice and facilitate shared learning to develop services and initiatives that meet local needs, in relation to aims and objectives of Wirral Community Health and Care NHS Foundation Trust.

Salary Expectations: The Trust pays staff in line with national NHS Terms and Conditions. Relevant experience may be considered when salary offers are made.
